Colorado native here who immensely loves driving, and has worked a range of food and beverage, retail, a few non-cdl delivery jobs, construction, distribution and general labor jobs. Driving is something that both excites and calms me, and I am looking forward to a new challenge and a career.
In my spare time, I enjoy writing, slowly learning new languages, videogaming, doing odd art, playing D&D or Pathfinder with friends, and making weird voiceovers for my cat.
I also enjoy listening to stories of other people's lives, especially related to travel, and thinking of new ways to make people laugh or smile.

So many documents for DOT and CDL school...where to start?
Hello,
I was wondering if anyone has experience with
1) Doctors Medical Opinion Release letters regarding past mental health history, and/or
2) Notarized or non-notarized letters from friends, others that state whereabouts during unemployment, multiple jobs, etc.
I am really hoping to get on with Millis Transfer, and if not then my second choice would be Prime. They both have locations easily accessible for training, pet policy for my cat once solo, etc. that fit in with what I need.
Long story short, the main thing I am trying to figure out what forms to provide for, is past mental health history. Through several mis-diagnoses, a hospital stay in an institution in 2021 for a week at the end of May to early June, there is a lot to account for. I have held a job for 6 months since October of last year, do not take medication past general OTC for pain, sleep, vitamins etc. and current psychiatrist thinks past meds may have caused a lot of the issues I have had.
I have a clean background, good driving records (I have also been doing non-CDL delivery driving as well here and there for the past 6 years in between jobs) and can pass any drug test given as well as random tests. I have not recently been on any medication for mental health other than for anxiety (Guanfacine for social anxiety at work) but those are not going to be refilled as I feel like I need to clear my system of past meds and do not have enough of a need for them.
My question is, what documents and supporting evidence is needed to bring to DOT physical?
What about other documents to support work history and any gaps? (Homeless 2x before and a few other gaps) For this, I am currently just trying to get letters and contact info from friends or others who knew my activities during unemployment, homelessness, etc.

Self Studying for CDL permit and skills test
Hi! I'm Indiana.
I just recently joined the site after a few years of looking into trucking, CDL companies, helpful posts here, and feedback from truckers I've talked to. I am ready to start my journey, and have been putting in applications like crazy to any companies that allow cats upon going solo.
I picked up the CDL Manual for NC yesterday, started studying it and taking notes, as well as going over YouTube videos and guides of skills test, truck parts etc. I was a bit indifferent about everything, but after I started studying I realized I am SUPER EXCITED for all the challenges and new skills I'll learn. I absolutely love driving, and have had a lot of non-cdl driving jobs in food delivery, transportation of people as a caretaker, and several odd driving jobs here and there.
Anyways, I just wanted to start off by saying hello, a little about me, and that I hope to learn a lot from folks on here as well as on the road.

Yep, I would do that option if possible and pay my own way and take my time getting my CDL. I've run into companies that don't have past employment history anymore, so I have no idea what to put there. Can't even estimate the dates for most things, either. I didn't even think about the TWIC check, but I wonder if that will help show where I was at in places that required fingerprinting like for government assistance. Hmm. I haven't done this much paperwork since high school, but I'm definitely more excited about this opportunity. I really hope they don't need notarized statements...
