Mark, if you fail the physical you get sent home to correct whatever it is that caused your fail. I had some medical issues when I first started trying to get hired so I know how this works. Usually they will let you return once you've taken care of the problem.
Mark, theres different rules on diabetic drivers. If you are diabetic, and just take the pills, you can drive Interstate...meaning all over the place. If you turn to injections for diabetic control issues, you will lose your CDL for interstate driving. now there are some states that allow injection dependent drivers to drive commercial vehicles within the state ...I'm not sure which states are that way, right now. The feds are also doing a pilot project on diabetic drivers, and they are giving waivers on a case to case decision. Since one of the more serious effects of diabetes is passing out, you can appreciate the reluctant attitude of the DOT....So you will need to start eating healthier, EXERCISE, and monitor your blood sugar levels, so you will know up front if you can pass that part of your DOT physical. Once I have the physical from the company sponsored training , if the Doctor finds that I am diabetic (I wasn't the last time I checked), will he write a prescription or will I fail the exam, be sent home, and then have to be checked by my personal physician and get medication before I can return to take the DOT physical?

Company Sponsored Training:
A Company-Sponsored Training Program is a school that is owned and operated by a trucking company.
The schooling often requires little or no money up front. Instead of paying up-front tuition you will sign an agreement to work for the company for a specified amount of time after graduation, usually around a year, at a slightly lower rate of pay in order to pay for the training.
If you choose to quit working for the company before your year is up, they will normally require you to pay back a prorated amount of money for the schooling. The amount you pay back will be comparable to what you would have paid if you went to an independently owned school.
Company-sponsored training can be an excellent way to get your career underway if you can't afford the tuition up front for private schooling.
