You can no longer get your permit with all endorsements. You can get a copy of your MVR at your local DMV or get one online.
The state agency that handles everything related to your driver's licences, including testing, issuance, transfers, and revocation.
An MVR is a report of your driving history, as reported from your state Department of Motor Vehicles. Information on this report may include Drivers License information, point history, violations, convictions, and license status on your driving record.
And for your physical, you must use a medical examiner who is certified by the FMCSA:

Yes I have started the program and will be doing more as I get closer to school starting. Told my wife starting September 1 I will be devoting 3-4 hours per day specifically to tests and studying. I currently read my CDL manual some everyday but will be going full force as I get closer to the day. I been doing online tests of other sites as well as TT plus all the great journals I been able to read on here.

What all is involved when I go to the DMV to get my CDL permit to start trucking school. Looking to register for school on Thursday. If I remember correctly before I start school I need to provide Driver history report and have permit with all endorsements I am pursuing as well as DOT physical completed.
Also need hazmat security threat assessment. What is that as well and where do I get that
