Getting my cdl back after a failed DOT drug test

I got my cdl back in 2014 and was driving for a year, in 2016 was in my early 20s at the time and partied alot and end up failing my DOT drug test it's been 5 years since then a did not do the SAP program due to not having insurance and not being able to afford it, I do wanna get back into truck driving, but I also had a DUI last year when I had my regular driver license but yet they still let me keep my license, my question is what do I have to do to get back in to driving again? TIA
