DOT physical question

Hello all! About 4 or 5 years ago I went to get a DOT physical and I stated that I had sleep apnea (i had no idea that was so serious to them) so I only got a 3 month pass with the requirement to go get another sleep study done. I never did and just moved on to something else. I am a veteran wanting to go to Swift driving school (free for vets). The first physical was in TN, but I'll be obtaining my CDL from Swift in MT or ID.

My question is: will that TN physical carry over and the DOT will be able to see it, or will I be fine getting my DOT physical in a different state several years later (without mentioning sleep apnea this time)?
