Why? Many states required that years ago when states had to verify all that stuff because of a new law they had enacted. .. I know Illinois did anyway. Since then, I've never had a drivers license agency require any of that, didn't even have that required for FBI and state background checks I was required to have done due to my previous profession. Maybe you live in a state that never complied with the verification laws? Idk.
I also have had federal background checks that didn't require all this extra identification. I was very surprised when the DMV asked for all this. Minnesota just started doing it.
Requirements effective May 4, 2015:
Beginning May 4, Minnesotans will ALSO need the following in order to apply:
Proof of citizenship or permanent residency (U.S. passport, birth certificate, permanent residency card). If the name on your U.S. passport, birth certificate or permanent residency card has changed you must also present proof of your legal name change(s). Acceptable proof consists of certified marriage certificates, certified divorce decrees or other certified court orders. Divorce decrees or other court orders must specify the name change. Your identity and name change documents need to show a clear link between your citizenship or permanent residency document and your current name.
The state agency that handles everything related to your driver's licences, including testing, issuance, transfers, and revocation.
Operating While Intoxicated
Check out the Real ID act of 2005. I see Minnesota is still not compliant. I have been a resident of a fully compliant state so I should be ok.
I've checked and Kentucky doesn't apoear to require anything of me other than my current class d license, dot medical card, cdl application, and a self-cerification form basically stating I will be driving interstate so we'll see.

Your very lucky. I spent a whole day hanging around our government center waiting in lines to get all the documents I needed. I'm glad it's over and I start school on the 8th. Woo hoo!

I'm putting this out there for women who are looking into truck driving. Before trying to take your permit test you will need to show a clear line of name changes. Bring the following to the dmv and you won't have any issues 1. Current license 2. Social security card 3. Health certificate in my case mndot exam 4. Birth certificate 5.Certified copy of marriage license 6. Certified copy of divorce papers, make sure it shows in the papers if you changed your name, this is very important! You must have paperwork for each and every marriage and divorce no matter how long ago it happen. I just passed my permit test and endorsements and start school on Sept 8. I am super excited
