On your card it should say CDL yes or no beside it should say only for interstate. Yes or no if its marked no then its not for your CDL

There is not an option for that on the card. I better make sure to check it out on Monday. It's kind of weird. My examiner was approve as well so it's kind of strange that the card doesn't have it.

Don't know if the cards change again for 2016 - the Long Form Questionnaire changed radically.
Some of the cards vary in appearance (depending on which supplier printed the out, Keller/etc.) but they all must contain the info found here:

You also want to make sure your licensing states DMV has your current MED CARD OR RENEWAL ON FILE. If the med card your DMV has on file EXPIRES - your license will automatically be DOWNGRADED TO A NON-CDL. If this happens and you're not aware of it (as in out OTR and didn't get to see the notice in the mail from DMV), and you get pulled over driving a CMV with a downgraded license, it's the same as NOT HAVING A CDL - even if you have the current DOT Med Card in your wallet.
I've heard of a few drivers getting jammed up for this - MAKE SURE your state DMV has a current, un-expired DOT Med Card on file.
Florida DMV (for example), you can upload a photo of the card to the DMV website - they will review and approve it and notify you via email that your med card was accepted, so you don't have to get back into a Florida DMV office to update your Med Card. Most states probably have a similar online facility, or you can mail a copy of your card in. Either way you do it - make sure you keep your state DMV up to date.

I dont think interstate or intrasate is on the new card..in the bottom right corner there is a yes and no circle that needs to be checked yes for cdl holder

Well I am all set with paper permit at Swift, now my worry is that it says to make sure CDL is check marked on med card. What exactly does the med card look like? Is it the med card the medical examiner certificate? Because if it is there is not any box for CDL to be check marked.
