ALL commercial drivers (CDL Holders) require a valid Medical Certificate.
That includes Class A, B & C.
By "interstate" do you mean ONLY ONE SINGLE STATE? That's actually INTRASTATE.
Either way - if you are operating a CMV that requires a CDL - you need to have a DOT Medical Certificate.
Check here on FMCSA's site for whether or not the type of driving you will do, requires a valid Med Card.

I can't seem to find a clear answer anywhere. Some of these sites are hard to navigate. I have a scheduled appointment for a DOT medical exam, but it's expensive. I am going to school for CDL B license, and I only want to drive interstate. Do I still need to get an exam?
