Can you clarify what are "B restrictions"? Any class can have restrictions of one kind or another, but I don't know of any Class A restrictions where you are forced to drive like a B.

Correct. I have the Class A CDL but I will only test in a dump truck with a six foot trailer. I would have to test in a semi to get the restrictions off.
A CDL is required to drive any of the following vehicles:
The definition of classes is by weight, not by length.
Any combination of vehicles with a gross combination weight rating (GCWR) of 26,001 pounds or more provided the GVWR of the vehicle(s) being towed is in excess of 10,000 pounds
The trailer name plate will show the GVWR. If your trailer does have that GVWR greater than the 10k, but is connected to the tractor (the "truck") with a pintle hitch, you can get the Class A CDL , but you are restricted from driving Class A with a fifth wheel.
As for upgrading, you will probably have to take a course including the fifth wheel (pre-trip and all that) plus take the road test again.
A CDL is required to drive any of the following vehicles:
GVWR is the maximum operating weight of a vehicle as specified by the manufacturer, minus any trailers.
The GCWR refers to the total weight of a vehicle, including all trailers.
