I had got 1 "fix it" ticket at the N.Mex scale, the right tail light was out on the trailer. After pulling out of the building to park, I fixed the light, went inside to have them sign it off. They could see, the light working, good to go. As soon as I'm walking back to the truck,I noticed the dang light had quit working again! hahaha

Had to re-adjust/tighten up the pigtail AGAIN, and was I off.....Company didn't get/LOST the scanned copy I sent of it being signed off, to do their part....Couple days later they're bugging me for it, so I had to resend it to them, done deal (crst)

Ok that is the inspection report. It shows no citation issued. Repair must be completed prior to his next dispatch and the inspection report with the mechanics signature must be sent back to CHP within 15 days.

Is there a specific form the mechanic fills out and signs?
