Good question Phillip. I've been wondering about this myself.
If it was done by a Swift Approved Clinic you won't have to. Otherwise they will make you take another one. Also if any info on that card is incorrect they won't accept it & you will have to take it over. I had my own done by an approved clinic which they recommended but once I got to Salt Lake City, Utah & they checked everyone's information mine didn't have the box checked for CDL A. I can't quite remember what it says but I assumed that it asked for your current license class not what what you were applying for. So I think my had the "other" box checked because I had a class "E" license which wasn't listed. But come to find out that CDL A should have been checked. It wasn't clear on the paperwork that you should put "A" it just looks as if they wanted you to put your current license status down.

So I'll be starting Swift training soon in Memphis I already got my DOT done will I have to do another one when I get there?
