Jerico, I just had my DOT physical with a Swift company doctor. Besides the usual BP, vision, etc., your physical should include some basic range-of-motion checks, like hold your hands/and above your head, pushing arms & legs against the Dr pushing back.
Nothing about squatting, lifting or even bending over. I suppose if you don't have a problem lifting 50 lbs off the floor to waist height you will be OK.
Some companies do further agility tests, like "duck walking" (getting under a trailer), but your recruiter can fill you in on this details.

Prime gives agility tests...the duck walk, climbing a ladder, raising arms, lifting different weights. Admit to any issues you have and it didn't seems a problem even for people with limited mobility in some aspects.
