I am not completely familiar with the process of a SAP eval but I would do whatever they tell you to do so that you can prove that you are not using any illegal substances. That is basically the only way that you will be able to make things happen. Even a refusal to take a drug screen can create huge problems for a CDL driver because the FMCSA does not mess around when it comes to this stuff. Good luck with the process.
A CDL is required to drive any of the following vehicles:
The CSA is a Federal Motor Carrier Safety Administration (FMCSA) initiative to improve large truck and bus safety and ultimately reduce crashes, injuries, and fatalities that are related to commercial motor vehicle
The FMCSA was established within the Department of Transportation on January 1, 2000. Their primary mission is to prevent commercial motor vehicle-related fatalities and injuries.
What Does The FMCSA Do?
The Substance Abuse Professional (SAP) is a person who evaluates employees who have violated a DOT drug and alcohol program regulation and makes recommendations concerning education, treatment, follow-up testing, and aftercare.

State and Federal DOT Officers are responsible for commercial vehicle enforcement. "The truck police" you could call them.
We have a lot of information on DOT drug testing policies and the return to duty process:

You said you received your CDL over a year ago. First and foremost you need to double check to see if your CDL has been downgraded to a class C license. If you do not maintain a current medical card and have self certified with DMV your CDL status has more than likely been downgraded and you will have to complete the CDL process all over again. There are exceptions in the self certification in which you can maintain your CDL with out a valid medical card.
You need to get a copy of your DAC report at www.hirerite.com and see if CRST reported to them that you were terminated for illegal drug use. Your DAC report is like a credit report and information on the report will remain for a period of 7 years. Any company you apply with will pull the report. You can fight inaccurate information on the DAC report. If CRST reported that you were terminated for illegal drug use and was not backed up by a DOT approved drug test, which tested positive or was a refusal, you have a valid claim against CRST and need to file a complaint with the Department Of Labor. Most states have a 2 year statute of limitation in which to file the complaint.
The SAP program does not apply to you. It is a program for current employee's of a company that (voluntarily) admit they have a substance abuse problem. The employee will be removed from all safety sensitive functions . In other words you can not drive a CMV. You will be sent to a substance abuse counselor for a complete substance abuse evaluation. Then you will have to submit to scheduled and random DOT drug screens for a period of one year. All of this will be a very expensive out of pocket expense as the employee is responsible for all costs not the company. All trucking companies that I know of have policies in place for zero tolerance substance abuse and if you test positive or refuse a DOT approved drug screen you will be immediately terminated. If terminated from a company for a positive, adulterated or refused DOT drug screen your chances of being re employed as a truck driver will be zero to none.
As far as your failed non DOT drug screen 9 or so years ago you were being honest in reporting it and I commend you for that. But you did not have to provide that information while in orientation. You do not have to list the job where you failed the pre employment drug test on any application as you were not hired and employed with the company.

A CMV is a vehicle that is used as part of a business, is involved in interstate commerce, and may fit any of these descriptions:
BMI is a formula that uses weight and height to estimate body fat. For most people, BMI provides a reasonable estimate of body fat. The BMI's biggest weakness is that it doesn't consider individual factors such as bone or muscle mass. BMI may:
It's quite common, especially for men, to fall into the "overweight" category if you happen to be stronger than average. If you're pretty strong but in good shape then pay no attention.
The state agency that handles everything related to your driver's licences, including testing, issuance, transfers, and revocation.
A truck drivers DAC report will contain detailed information about their job history of the last 10 years as a CDL driver (as required by the DOT).
It may also contain your criminal history, drug test results, DOT infractions and accident history. The program is strictly voluntary from a company standpoint, but most of the medium-to-large carriers will participate.
Most trucking companies use DAC reports as part of their hiring and background check process. It is extremely important that drivers verify that the information contained in it is correct, and have it fixed if it's not.
