It expires every 5 years or when ever your license expires which ever comes first. You have to get finger printed and take the test again to renew it.

Don't you have t get anther background check completed too? Had to have it done when I first got mine. Letting mine expire as my employer does not handle hazmat freight.

Yes, you do need another background. That's part of the process.
