On my 3rd day at the Swift Academy in Waco, Ga. 2 days ago. I and another student had to re-do our DOT physicals owing to a clerical error committed by Concentra Clinics they said. No problem. I passed again as I knew I would. Already had the 1 year card; 1 year because of type 2 diabetes. To get that card had to provide A1C and all types of other documentation from my own MD. Fine. Did that. As I was filling out yet another Med Exam questionnaire I omitted the part about type 2 diabetes seeing how we had already gone through that just a few weeks before and Swift and FMCSA were fully apprised of that condition. When I arrived back at the academy from a local clinic both the academy director and coordinator were waiting for me. They were quite livid and pointed out the discrepancy between the 2 physicals taken just weeks apart. They would accept no explanations from me. Really, there was nothing I could say. They both further stated I was guilty of making a false statement on the form and they ejected me from the school.

CSA:

Compliance, Safety, Accountability (CSA)

The CSA is a Federal Motor Carrier Safety Administration (FMCSA) initiative to improve large truck and bus safety and ultimately reduce crashes, injuries, and fatalities that are related to commercial motor vehicle

FMCSA:

Federal Motor Carrier Safety Administration

The FMCSA was established within the Department of Transportation on January 1, 2000. Their primary mission is to prevent commercial motor vehicle-related fatalities and injuries.

What Does The FMCSA Do?

  • Data and Analysis
  • Regulatory Compliance and Enforcement
  • Research and Technology
  • Safety Assistance
  • Support and Information Sharing

DOT:

Department Of Transportation

A department of the federal executive branch responsible for the national highways and for railroad and airline safety. It also manages Amtrak, the national railroad system, and the Coast Guard.

State and Federal DOT Officers are responsible for commercial vehicle enforcement. "The truck police" you could call them.

Fm:

Dispatcher, Fleet Manager, Driver Manager

The primary person a driver communicates with at his/her company. A dispatcher can play many roles, depending on the company's structure. Dispatchers may assign freight, file requests for home time, relay messages between the driver and management, inform customer service of any delays, change appointment times, and report information to the load planners.

OWI:

Operating While Intoxicated

I don't know what to write other than information on those forms is very important. Unfortunately, they gave you no second chance on that. Pay very strict attention on your next opportunity.

HOS:

Hours Of Service

HOS refers to the logbook hours of service regulations.

This is a classic example of what can happen if you do not read and follow the instructions on the medical forms. Do not assume anything.

No where on there (the firm) does it state it’s okay to omit anything regardless of how many times it’s necessary to repeat the process.

Apply everywhere and follow the rules. Also understand you may take multiple physicals over time and will have to provide these documents ever time. I know someone who took 3 physicals in 5 weeks. It happens.

Terrible learning experience.. but hang in there.

For information, every DOT physical is brand new and has nothing to do with your last one. If you take one this morning and pass, then another this afternoon and don't pass. The most recent one is what counts. It is very important to fill them out the same. This is a federal legal document, any missing or falsified info will be problems.
